Ms. Bernie, the Health Technician provides first aid and emergency care to students and maintains cumulative health and immunization records for all students. In addition, the health technician can help students obtain health-related resources available in the community.

INSTRUCTIONS FOR MEDICATIONS

It is important to ensure that the following requirements are met regarding medication administration for students who attend Evergreen Valley High School:

1. The necessary forms must be completed by a licensed healthcare provider in the State of California, as well as by the student's parent or legal guardian.

2. These forms must be renewed every academic year that the student attends Evergreen Valley High School, or when there is a change in medication dosage or Medical Health Plan.

3. For students who are required to carry and self-administer emergency medication such as EpiPen, Insulin Pen, or Inhaler on campus, they must have an "Authorization to Carry and Self-Administer Emergency Medication on Campus" form, along with an authorization form on file in the Health Office.
